Problems with Ezoflash 4v4

Hi everyone!
I have built Ezoflash 4v4 in my atempting to flash an ASRock motherboard bios. The chip is W49F002UP. However a have some problems with it. I made PLCC32 adapter for it. Without chip when clicking on GET ID i receive 0x40 SyncMosel. With chip on socket i get 0x00 and no brand.
I did a hardware test and i found that no voltage is present on A10 nor on IC3 pin 11.Also there is a problem with VCC-32: when enabled i have 5v, when disabled 1,40V. Does anyone know what it could be?

IC used:IC4,IC5: MMC4503
IC6:MMC4014
IC7: 74ls06
IC1,IC2,IC3: CD4015.
V2:2n5401 (pins orientated accordingly), V4: BC308.
